Summary
The Primary Care Provider (Physician) is responsible for assessment and care of new and returning patients with multiple medical problems, ordering diagnostics, consultations, medications and other therapies as medically indicated on an urgent need basis, meeting preventative health maintenance needs, completing all required VA computer documentation including clinical reminders and screenings, adhering to all VA Clinical Practice Guidelines elements.
Responsibilities
Functions as the Physician, Patient Aligned Care Team (PACT), Primary Care Services, Community Based Outpatient Clinic (CBOC) setting at Montana VA Healthcare System (MT VA HCS). Responsible for providing medical care in an outpatient setting following PACT principles. May provide medical care using Telehealth for remote patients or travel to other CBOCs to cover provider shortage. Participates in multi-disciplinary team meetings to achieve a patient centered, holistic approach for clinical care and needs of the Veteran, focusing on continuous improvement of critical team processes. Ensures the patient's care plan contains medical recommendations for clinically indicated care. Ongoing, continuous care of one or more assigned panel(s) of Veterans providing health education and health coaching on wellness, disease prevention, chronic care management and self-management skills to patients and personal support persons. The incumbent must be able to work collaboratively with ancillary staff including nursing and clerical staff and on-site Mental Health Providers.
Work Schedule: Monday - Friday; 8:00am to 4:30pm or as agreed upon with possibility of extended hours and weekends. NOTE: Candidates must be willing to work weekends, irregular work hours and extended hours. The candidate must be willing to be assigned to other related services and/or locations, if necessary.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.
Proficiency in spoken and written English.

Physical Requirements: This position requires a full range of physical activity, including lifting and carrying, pulling, pushing, reaching at and above the shoulders, use of both hands and fingers, walking, standing, sitting, kneeling, repeated bending, and ability for rapid mental and muscular coordination simultaneously. Adequate hearing (aid permitted), mental and emotional stability required.
