Home Based Primary Care is a program which brings a primary care and interdisciplinary team to the home for elderly, frail veterans with complex medical problems.
Responsibilities
The HBPC Medical Director works collaboratively with Program Managers to provide leadership to the HBPC program. This program follows a PACT model and includes primary care providers, social workers, RN's, LPNs, rehabilitation staff, dietitians, pharmacists and psychologists who make in home visits and collaborate in the care of this high risk high needs population. The main site is in White River Junction, Vermont; with teams in Bennington, Vermont; Brattleboro/Keene/Burlington, VT; and Littleton, NH.
The HBPC Medical Director:
Practices in the Home Based Primary Care Program of the White River Junction Veterans Health Care System including in the homes of the veteran clients.
Is academically and clinically prepared to supervise/consult and perform a broad spectrum of patient care services at the expert level.
Provides clinical input and oversight for care of all patients; and is available to all team members for clinical collaboration; and
Serves as a consultant to nursing and medical staff for patients with complex treatment or care needs in the home.
Collaborates with Program Manager and GEC leadership in strategic planning; and with the Program Manager in hiring personnel, developing policy and standards of practice and in performance improvement.
Works with the Program Manager to plan and direct the educational and clinical experience of learners; and works with the program manager to monitor the program to ensure quality of care and success of performance measures.
Provides geriatric or palliative care consultation on specific patients in the home as indicated; certifies hospice referrals and trouble shoots issues with community services; and educates staff regarding updates on state requirements for death certificates or hospice certification.
Collaborates with Program Manager to Plan and direct the educational and clinical experience of learners includes planning for medical residents, medical students, PA students, RN students, NP students. Teaching includes making home visits with learners and supervising the visits of learners with other staff.
The HBPC Medical Director will have a faculty appointment at Geisel School of Medicine and will be involved in academic activities.

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 05/27/2019.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.
Proficiency in spoken and written English.
Physical requirements outlined below.
Preferred Experience: Board certified Geriatrician preferred.
Driver License: The position requires that the incumbent possess a valid driver's license.
Physical Requirements: The incumbent must by physically able to drive to the client's homes and to conduct clinical evaluations in the home.
Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-2 Physician Qualification Standard. This can be found in the local Human Resources Office.
